What is the MI Choice Waiver Program?

The MI Choice Waiver is a program that provides additional services and support to individuals who are eligible for Medicaid but would otherwise require institutional care. The Waiver program allows eligible individuals to receive care and services in their home or community settings, rather than in a nursing home or other institutional facility.

What are MI Choice Eligibility Requirements?

  • Adults age 65+
  • Adults age 18-64 and certified disables
  • Must Meet Michigan Medicaid nursing facility level of care criteria
  • Individual income no higher than $2,742/month
  • Assets less than $2,000 for a single person
  • Minimum protected amount if the client has a spouse is $29,724. Maximum protected amount is $148,620
  • Qualifying individuals may own a home, vehicle and have no more than $2000 in savings.

Because this process is time consuming, it can take up to four to six weeks to have your Medicaid application approved and get your assigned Medicaid  number.  It is recommended to apply as soon as you are eligible as stated above.

As you complete the application, please note the following:

  • You may put in your relative’s address and then, in later place, put a separate mailing address. This will help you to receive and return necessary documents.
  • You will be required to enter either a cell phone number or email address, which will be verified immediately before proceeding to the application.
  • There is a section midway that asks if anyone has had income, select: “Yes” and put in his/her gross income (source will likely be Social Security and/or pensions or VA).

You should receive a Supplemental Questionnaire in the mail about a week after application. Be sure to complete this and send it back in with a copy of the most recent bank statement (showing under $2000) and any other proofs requested. Also, this letter will have a DHHS specialist listed on it, this is a good place to start with any questions about the application.

Applying for Disability 

In Michigan an individual must have a physical or mental condition that prevents them from being able to work and is expected to last at least one year or result in death. The individual must also meet certain financial criteria such as having limited income and resources. The state of Michigan also has specific requirements for age, residence and citizenship.

The Social Security Administration (SSA) is the federal agency that processes disability claims in Michigan. To apply for disability benefits individuals must complete an application which can be done online, in-person or over the phone. The application will ask for medical and financial information as well as other personal details.

Once the SSA has received the application it will review the information and make a determination about eligibility for disability benefits. If the individual is approved they will receive a monthly benefit based on their disability.
